1. What is the FreeStyle Promise Program?
  The FreeStyle Promise Program is a free program that provides you with the knowledge and support to help you better manage diabetes. As a member, you may be eligible for instant co-pay savings on test strips every month.* In fact, eight out of ten members with private insurance pay only $15 every month for test strip co-pays.* As a member you will also get:
  - Free access to a team of licensed Certified Diabetes Educators^‡
  - Guidance from Insurance Consultants to help you understand how to best use your coverage for your diabetes testing supplies
  - A lifetime of free meter and battery replacements^†
  - 24-hour technical support and live meter training
  - Quarterly e-Magazines filled with helpful advice
  - Special offers
2. Can anyone enroll in the FreeStyle Promise Program?
  
  The FreeStyle Promise Program is currently for United States residents only.
  
  To enroll online at MyFreeStyle.com, you must be 18 years of age or older. If you are under 18 years of age, please call the FreeStyle Support Team at 1-866-246-2683 to complete your enrollment.
3. How much does it cost to join the FreeStyle Promise Program?
  
  The FreeStyle Promise Program is a free benefit. There is no cost to join or to maintain your membership.
4. How is the Freestyle Promise Program funded?
  
  The FreeStyle Promise Program is fully funded by Abbott Diabetes Care.
Getting Started
1. How do I sign up for the FreeStyle Promise Program?
  You have two options for signing up for your free FreeStyle Promise Program membership:
  - Online - please visit our online registration form. This option is available 24-hours a day.
  - Over the phone - please call the FreeStyle Support Team at 1-866-246-2683 during the hours of 7:00 AM and 9:00 PM Central Standard Time.
2. When will I receive my FreeStyle Promise Program membership number so that I can begin saving on my co-pay costs?
  When you enroll online:
  - Your membership number will be emailed to you three to four days after you complete enrollment, if you opt in to receive communications.
  - To begin saving on your monthly co-pay costs*, print the email with your membership number and bring it to the pharmacy along with:
    
    Your prescription for FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips or Precision Xtra Blood Glucose Test Strips, and
    
    Your insurance card (if applicable).
  - You will receive your actual membership card in the mail within two to three weeks.
  When you enroll over the phone:
  - Your membership number will be emailed to you three to four days after you complete enrollment, if you provide your email address and opt in to receive communications.
  - To begin saving on your monthly co-pay costs*, print the email with your membership number and bring it to the pharmacy along with:
    
    Your prescription for FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips or Precision Xtra Blood Glucose Test Strips, and
    
    Your insurance card (if applicable).
  - You will receive your actual membership card in the mail within two to three weeks.
  If you need your membership number right away, please call the FreeStyle Support Team at 1-866-246-2683 during the hours of 7:00 AM and 9:00 PM Central Standard Time.
3. What type of information will be emailed to me?
  With your permission, Abbott Diabetes Care will email you relevant information to help you manage your diabetes, including:
  - Your FreeStyle Promise Program membership number. Your membership number will be emailed to you three to four days after you sign up for the FreeStyle Promise Program so you can begin saving on your co-pay costs* right away - before your actual card arrives in the mail.
  - Quarterly e-Magazines filled with helpful advice, personal stories and more—delivered right to your inbox.
  - Information after you speak to a FreeStyle Promise Program Associate and/or a Certified Diabetes Educator that will include links to helpful websites and answers to your questions.
  - Information about free meter replacements^† and other special offers.
  To update your email preferences, please log in to your account at MyFreeStyle.com and go to "My Account."
4. Can I still enroll in the FreeStyle Promise Program if I choose to opt out of email and phone communications?
  
  Yes. However, please keep in mind that if you choose to opt out of email communications, then you will not receive updates about free offers and new tools when they become available. For example, you will not receive our quarterly e-Magazine, and you will not be notified if you become eligible for a free meter replacement.^†
Co-pay Savings on Test Strips*
1. How much can I save on the co-pay for my blood glucose test strips?
  
  If you are eligible for the co-pay savings benefit, you can use your FreeStyle Promise Program membership card to receive up to $25 off on a box of 50-count strips or up to $50 off on a box of 100-count strips every month.
  
  You are responsible for the first $15 of each co-pay under your insurance coverage, and the FreeStyle Promise Program will cover additional co-pay charges up to $50 every month and up to $600 per year. Eight out of ten people with private insurance pay only $15 per month when they use their FreeStyle Promise Program membership card.
  
  The co-pay savings benefit is good once a month, every month.
2. How can I get my co-pay savings on test strips?
  To begin saving on your monthly co-pay costs, bring the following to your pharmacy:
  - Your prescription for FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips or Precision Xtra Blood Glucose Test Strips.
  - Your insurance card (if applicable).
  - Your FreeStyle Promise Program membership number. This number will be emailed to you three to four days after you enroll, if you provide your email address during enrollment. Your actual membership card will arrive within two to three weeks.
  If you need your membership number right away, please call the FreeStyle Support Team at 1-866-246-2683 during normal business hours.
3. Do I need a prescription even if I am buying test strips over-the-counter?
  
  Yes, please ask your doctor or pharmacist for a prescription for FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips or Precision Xtra Blood Glucose Test Strips to get co-pay savings on your test strips. You can also use a generic (non-branded) test strip prescription and receive the co-pay savings.
  
  You will also need a prescription for a FreeStyle Lite®, FreeStyle Freedom Lite®, Precision Xtra®, or generic (non-branded) meter in order to get your free meter at the pharmacy.
4. Am I eligible for the co-pay savings on test strips?
  
  If you have private insurance (e.g. a PPO or HMO plan), or if you pay out-of-pocket for your test strips, then you should be eligible for the co-pay savings benefit. Among FreeStyle Promise Program members with private insurance, eight of ten people pay only $15 per month on test strip co-pays.
  
  If you are currently enrolled in Medicare, Medicaid or similar Federal and State program, you are not eligible to receive co-pay savings from the FreeStyle Promise Program.
5. I have Medicare, Medicaid or other similar Federal and State insurance. Why am I not eligible for the co-pay savings on test strips?
  Medicare, Medicaid and similar Federal and State insurance plans determine the price their customers pay for test strips and do not allow for additional co-pay savings.
  
  You can still take advantage of all the other free benefits of the FreeStyle Promise Program:
  - Free access to a team of licensed Certified Diabetes Educators^‡
  - Guidance from Insurance Consultants to help you understand how to best use your coverage for your diabetes testing supplies
  - A lifetime of free meter and battery replacements^†
  - 24-hour technical support and live meter training

6. I get my supplies through the mail. Can I still take advantage of the co-pay savings benefit?
  Yes. If you receive your FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips or Precision Xtra Blood Glucose Test Strips in the mail and would like to take advantage of the co-pay savings benefit, please follow these steps:
  - Call the FreeStyle Support Team at 1-866-246-2683 to request a Rebate Form.
  - When you receive the Rebate form, fill it out completely and mail it to the address included on form. You must include your prescription/mail order receipt and your FreeStyle Promise Program membership card.
  - In 10 to 14 days you will receive a check along with an additional rebate for use toward future purchases.

7. Which test strips are covered under the co-pay savings benefit?
  The following test strips are valid for ongoing monthly savings:
  - FreeStyle® Blood Glucose Test Strips- Box of 50
  - FreeStyle® Blood Glucose Test Strips- Box of 100
  - FreeStyle Lite® Blood Glucose Test Strips - Box of 50
  - FreeStyle Lite® Blood Glucose Test Strips - Box of 100
  - Precision Xtra Blood Glucose Test Strips - Box of 50
  - Precision Xtra Blood Glucose Test Strips - Box of 100
8. Is the co-pay savings benefit good on other Abbott Diabetes Care products such as lancets?
  
  No. The co-pay savings benefit is for FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips and Precision Xtra Blood Glucose Test Strips only.
9. Why does Abbott Diabetes Care offer co-pay savings? What is the benefit to the company?
  
  Abbott Diabetes Care offers co-pay savings to help make the costs of managing diabetes easier for our customers.
  
  The co-pay benefit helps our customers obtain FreeStyle® Blood Glucose Test Strips, FreeStyle Lite® Blood Glucose Test Strips and Precision Xtra Blood Glucose Test Strips at the same price, or at a lower price, than the preferred brand through your insurance.
Free Meter^†
1. How can I get my free meter?
  
  Once you complete your FreeStyle Promise Program enrollment, you will be mailed a FreeStyle Promise Program membership card. To get your free meter, bring your membership card and your prescription for a FreeStyle Lite®, FreeStyle Freedom Lite® or Precision Xtra® meter to your pharmacy to receive your free meter.
  
  You can also bring a generic (non-branded) blood glucose meter prescription to the pharmacy and receive your free FreeStyle Lite®, FreeStyle Freedom Lite® or Precision Xtra® meter.
2. I am undergoing peritoneal dialysis. Which free meter should I select?
  
  If you are a person with diabetes who is undergoing peritoneal dialysis with icodextrin, please call the FreeStyle Support Team at 1-866-246-2683 to request a Precision Xtra® meter.

1. What is the phone number to call to talk with a Certified Diabetes Educator?
  
  To speak with a licensed Certified Diabetes Educator, please call 1-866-246-2683 between the hours of 7:00 AM and 7:00 PM Central Standard Time.
2. What are the hours of operation for the Certified Diabetes Educator helpline?
  
  The hours of operation for the Certified Diabetes Educator helpline are 7:00 AM and 7:00 PM Central Standard Time.
3. How much does it cost to talk to a Certified Diabetes Educator?
  
  There is no cost. The Certified Diabetes Educator helpline is a free benefit for members of the FreeStyle Promise Program.
4. What is a Certified Diabetes Educator?
  
  Diabetes educators are licensed healthcare professionals such as nurses or dietitians. They have extensive knowledge about diabetes that they have obtained through clinical practice experience, continuing education and individual study. Many diabetes educators also have a Certified Diabetes Educator (CDE) credential which demonstrates specialized knowledge in the field of diabetes education.
  
  Diabetes educators can help you better understand how to incorporate healthy self-care behaviors into your daily life.
